IgG  medierad autoimmunitet: Fc Glykosylering, Citrullin igenkänning, Initiering och genidentifikation i Artrit


Autoimmuna sjukdomar uppstår när kroppsvävnad felaktigt attackeras av sitt eget immunsystem. Reumatoid artrit (RA) är en autoimmun sjukdom som angriper lederna och leder till kronisk inflammation. Smärta, svullnad, stelhet och förstörelse av lederna är vanligt och kan leda till minskad rörlighet hos RA patienter. RA drabbar 0,5-1% av världens befolkning. Kvinnor drabbas oftare än män. Vad som orsakar sjukdomen och hur den utvecklas är oklart. Tidigare lyckade behandlingar av RA som har gjorts genom att med antikroppar ta bort B cellerna ger hopp åt lidande patienter. Tack vare framsteg i behandlingen av RA med bortplockande av B-celler har nytt intresse väckts för vilken roll som B-cellerna och antikropparna som de utsöndrar har i utvecklingen av sjukdomen. IgG antikroppar bidrar inte bara till inflammationen associerat med immunkomplex utan kan också ha en direkt nedbrytande effekt på brosket. Syftet med detta projekt är att förbättra vår förståelse för B-cellernas och antikropparnas roll i att framkalla RA vilket kommer att ha betydande inverkan på förmågan att utveckla nya behandlingar för RA och andra autoimmuna sjukdomar orsakade av IgG (t.ex: lupus nephritis, myasthenia gravis). Nyligen utnyttjade vi bakterieenzymet från S. pyogenes, EndoS för att specifikt klyva kolhydrater som finns på Fc delen av den artrit framkallande IgG molekylen och detta ger ett betydande skydd mot artrit. I detta projekt vill vi specifikt klyva Fc kolhydraterna direkt i mössen genom att injicera EndoS och undersöka vilken effekt det har för att förebygga artrit. RA patienter har också höga nivåer av citrullinantikroppar som används i diagnossyfte. Om dessa citrullinspecifika antikroppar har sjukdoms framkallande effekt är inte bevisat och inte heller vet vi hur dessa antikropps producerande B celler väljs ut. I detta projekt vill vi skapa och studera en ny transgen mus som bara producerar anti-citrulline antikroppar för att besvara dessa frågor. Dessutom är djurmodeller nödvändiga för att undersöka hur artrit utvecklas och vilka faktorer som orsakar sjukdomen? Men, djurmodeller som används idag för artrit studier använder ett själv-antigen (t.ex kollagen typ II) blandat med en substans kallad adjuvans för att framkalla sjukdomen. Dessa adjuvanser som används vid injektion stör alltid immunsvaret så att vi inte vet vilket det faktiska immunsvaret mot självantigenet är? I detta projekt vill vi utveckla en ny adjuvans genom att använda smarta, overksamma och väldefinierade kemiska polymerer för att förstå immunsvarets karaktär mot själv-antigen utan att involvera klassiska adjuvanser. Viktigast av allt, för att förstå sjukdomens utvecklande måste vi identifiera de gener som gynnar sjukdomen. I denna riktning kommer vi att identifiera minst en gen som gynnar antikroppsfrämjande artrit under projektperiod. Alltså kommer denna studie att ge resultat som hjälper till att bättre förstå RA sjukdom. Detta föreslagna projekt kommer också att leda till humana studier i denna riktning och eventuellt hjälpa till att utveckla nya effektiva behandlingar för RA patienter och patienter med andra autoimmuna sjukdomar inducerade av IgG antikroppar.
